systemische antimikrobielle Therapie
Infektion 1. Wahl (1) / Alternative (A) Anmerkungen
− starte mit empirischer Therapie: Aminopenicillin/Betalaktamase-Inhibitor i.v. (p.o. unzureichend)
− sobald Antibiogramm verfügbar, erfolgt gezielte Antibiotikatherapie (Deeskalation)
− Gesamtdauer der Therapie: 12 Wochen (meist 2 Wochen i.v., dann p.o.)
− Beachte Dosisanpassung bei Niereninsuffizienz und Körpergewicht < 40 kg bzw. >100 kg
− wichtiges biofilm-aktives Antibiotikum ist Rifampicin: nicht im prothesenfreien Intervall (sondern
erst nach Implantation der neuen Prothese), nicht bei offenen Wunden oder liegenden Drainagen
oder als Monotherapie (Resistenzentwicklung)
