EIGENSCHAFTEN:
− Foscarnet hemmt als Pyrophosphatanalogon virale DNA-Polymerasen direkt (ohne die virale
Thymidinkinase); daher auch bei Aciclovir-/Ganciclovirresistenz erfolgversprechend
− z.T. auch Hemmung reverser Transkriptasen (HIV)
− renale Elimination (CAVE: Niereninsuffizienz)
WIRKSPEKTRUM INDIKATION
Zytomegalievirus (CMV) lebens- oder augenlichtbedrohende CMV-Infektionen
bei HIV-Infizierten
andere Herpesviren keine Zulassung (schwere Nebenwirkungen)

NEBENWIRKUNG:
− Nierenfunktionsstörungen: Kreatininanstieg (1-10 %) bis zum Nierenversagen (=> auf
ausreichende Hydrierung achten); Elektrolytstörungen: Hypocalcämie, Hypokaliämie (> 10 %)
− sehr häufig (> 10 %): gastrointestinale Beschwerden, Fieber, Exanthem
− neurotoxisch: > 10 %: Kopfschmerzen, Schwindel, Parästhesien; 1-10 %: Myoklonien, Tremor,
Krampfanfälle
− Blutbildveränderungen: Granulozytopenie (> 10 %), Thrombopenie (geringere Knochenmarkschädigung
als Ganciclovir)
KONTRAINDIKATION:
− Gravidität / Stillperiode
− Kombination mit Pentamidin i.v. (Nephrotoxizität ↑↑, Hypocalcämie)
− Vorsicht bei Pat < 18 J (Erfahrungen ↓), schwerer Niereninsuffizienz/Dialyse
Neueres Virustatikum bei CMV-Infektionen, Alternative zu Ganciclovir.
